Planned Lifetime Assistance Network of Central Ohio (PLANCO) is the only nonprofit organization working in Franklin and the surrounding counties to provide future care planning and support services for the mentally and physically disabled. Our clients include the mentally ill and mentally retarded, as well as those with brain injuries and degenerative physical conditions. PLANCO was started in 1999 by a group of families whose main concern was who would care for their dependents with disabilities, often sons and daughters, when they were no longer able to do so themselves. Today, our mission remains the same: We provide a safety net of coordinated care and support for our clients both now and in the future, and peace of mind for families who can no longer provide that critical care and support.
PLANCO develops, implements and monitors a personal "Plan of Care." It covers every aspect of the client's life and incorporates services already provided by public agencies that will be monitored by PLANCO case managers. Working in tandem with public sector assistance, PLANCO provides an array of services and supports including escorted transportation to and from shopping and other appointments, social and recreational activities, and family surrogacy and advocacy. In essence, PLANCO bridges the gaps in care for the mentally and physically disabled. For those with mental illness, in particular, falling through those gaps can create a cycle of medication noncompliance, leading to hospitalization, loss of job and even incarceration and homelessness.
